What Is ARA-290?
ARA-290, also known as cibinetide, is a synthetic peptide derived from the tissue-protective region of erythropoietin (EPO). It was developed to activate the Innate Repair Receptor (IRR), supporting anti-inflammatory and tissue-protective signaling without stimulating red blood cell production.
Unlike conventional EPO, ARA-290 is designed to activate tissue-protective pathways without increasing hematocrit or hemoglobin.
ARA-290 has been studied for potential applications involving:
-
Small fiber neuropathy
-
Neuropathic pain
-
Nerve irritation and injury
-
Chronic inflammatory conditions
-
Sarcoidosis-associated neuropathy
-
Diabetes-associated small fiber neuropathy
-
Tissue protection and recovery
Research has focused particularly on its potential effects on inflammatory signaling, small nerve fibers, and tissue-protective pathways.
Why ARA-290 Matters
ARA-290 is being investigated for its potential to support nerve health and tissue protection in conditions involving inflammation or small nerve fiber dysfunction.
Potential Applications
-
Neuropathic Pain: Burning, tingling, hypersensitivity, and nerve-related discomfort
-
Small Fiber Neuropathy: Dysfunction involving small sensory and autonomic nerve fibers
-
Nerve Injury: Persistent nerve irritation following injury or tissue damage
-
Inflammatory Conditions: Conditions involving excessive inflammatory signaling
-
Tissue Protection: Supporting the body's innate tissue-protective pathways
-
Nerve Health: Investigational support for nerve function and quality of life
Clinical studies have reported encouraging findings in selected neuropathic conditions, although benefits are still being investigated.
🔬 Key Research-Based Mechanisms
🧠 Innate Repair Receptor Activation
ARA-290 targets tissue-protective signaling associated with the EPOR–CD131 receptor complex, also referred to as the Innate Repair Receptor. Activation of this pathway is associated with cellular protection and repair without the erythropoietic effects of conventional EPO.
Research: Brines et al., 2008.
🔥 Inflammatory Signaling Modulation
ARA-290 has demonstrated anti-inflammatory and tissue-protective activity in experimental research, potentially helping regulate inflammatory responses associated with tissue and nerve injury.
Research: Brines & Cerami, 2012.
⚡ Small Fiber Nerve Support
Human clinical studies have investigated ARA-290 in patients with small fiber neuropathy, including sarcoidosis-associated neuropathy, with research evaluating neuropathic symptoms and small nerve fiber integrity.
Research: Heij et al., 2012; Dahan et al., 2013.
🛡️ Tissue Protection & Recovery
ARA-290 has demonstrated activity within pathways associated with cellular survival, inflammation regulation, and tissue protection following injury.
Research: Brines et al., 2008.
💉 ARA-290 Dosing Protocol
10 mg ARA-290 + 1 mL PBS
After reconstitution:
10 mg ÷ 1 mL = 10 mg/mL
On a U-100 insulin syringe:
-
10 units = 1 mg
-
20 units = 2 mg
-
30 units = 3 mg
-
40 units = 4 mg
Standard Schedule
5 Days ON / 2 Days OFF
Administer once daily for 5 consecutive days, followed by 2 days off, then repeat.
Route: Subcutaneous injection
Typical Cycle: 4–12 weeks based on clinical goals, response, and provider recommendations.
Dosing Options
| Protocol | Dose | Injection | Approx. Vials / Month |
|---|---|---|---|
| Option 1 | 1 mg | 10 units | 3 vials |
| Option 2 | 2 mg | 20 units | 5 vials |
| Option 3 | 3 mg | 30 units | 7 vials |
| Option 4 | 4 mg | 40 units | 9 vials |
Monthly supply is calculated using approximately 21–22 treatment days per month under the 5-days-on/2-days-off schedule and is rounded up to whole vials.
How Long Does One Vial Last?
Each vial contains 10 mg total.
-
1 mg / 10 units: 10 treatment doses per vial
-
2 mg / 20 units: 5 treatment doses per vial
-
3 mg / 30 units: approximately 3.3 treatment doses per vial
-
4 mg / 40 units: 2.5 treatment doses per vial
The appropriate dose is determined through consultation based on treatment goals, clinical presentation, tolerance, and response.
💧 IMPORTANT: PBS IS REQUIRED
ARA-290 Must Be Reconstituted With the Supplied Phosphate-Buffered Saline
This formulation of ARA-290 requires 1 mL of the specific sterile phosphate-buffered saline (PBS) supplied with the order.
Do not substitute bacteriostatic water or another reconstitution solution.
Using an incorrect diluent may cause the ARA-290 to clump, aggregate, or form a gel-like consistency, compromising the vial and making the solution unsuitable for administration.
Proper Reconstitution
-
Use only the supplied sterile PBS.
-
Add 1 mL PBS to the 10 mg ARA-290 vial.
-
Follow the provided reconstitution instructions.
-
Inspect the solution before administration.
-
Do not inject a solution that develops clumping, gel formation, discoloration, or visible particles.
-
Refrigerate the reconstituted vial according to the supplied storage instructions.
⚠️ Product Replacement Policy
Use of the supplied PBS is required for product warranty coverage.
Vials compromised after being reconstituted with bacteriostatic water, sterile water, or another unauthorized diluent cannot be warrantied or replaced due to improper reconstitution.
If there is any uncertainty about which solution to use, contact PRO Peptides before reconstituting the vial.

Important Considerations
ARA-290 (cibinetide) is investigational and is not FDA-approved for the treatment of neuropathy, chronic pain, nerve injury, or inflammatory conditions.
Human research has demonstrated potential benefits in selected neuropathic populations, particularly small fiber neuropathy, but larger studies are needed to establish efficacy, long-term safety, and optimal dosing.
The 1–4 mg dosing options and 5-days-on/2-days-off schedule described above represent the PRO Peptides clinical protocol and should not be interpreted as an FDA-approved dosing regimen.
Medical consultation is required to determine whether ARA-290 is appropriate for an individual patient.
